Facilities Use Agreement — By submitting this form, the User agrees to abide by all applicable policies, indemnifications, safety requirements, and scheduling decisions set by Highway Tabernacle of Prayer. Insurance and compliance obligations are acknowledged by the User. The Church reserves the right to rescind or reassign space as needed.
